Residing at Yıldırım Neighborhood, 35th Street, No: 62-/- Menemen, İzmir, I acknowledge that I have read and understood the disclosure text and personal data protection policy of HASAT BNO GROUP FOOD MEAL LIVESTOCK TEXTILE CONSTRUCTION INDUSTRY AND TRADE JOINT STOCK COMPANY. In accordance with the Personal Data Protection Law No. 6698, I have been informed by the Company about the purposes for which my personal data will be processed, to whom and for what purposes the processed personal data may be transferred, the method and legal basis of personal data collection, and my other rights listed in Article 11 of the Law, leaving no room for doubt.

By the Company, in accordance with the relevant provisions of the Personal Data Protection Law No. 6698 (“Law”); I consent to the processing of my health data, which is classified as sensitive personal data, (i) to fulfill legal obligations, (ii) to conduct occupational health and safety activities, (iii) to ensure activities are conducted in compliance with legislation, (iv) to conduct storage and archiving activities, and (v) to provide information to authorized persons, institutions, and organizations. I also consent to the transfer of this data to the Company’s parent company and group companies, public institutions and organizations, persons, institutions, and organizations from whom services are received for these purposes, supplier companies due to the use of cloud computing infrastructure, and its retention for the necessary period. I am aware that I have the right to withdraw this statement at any time with the understanding of my rights as a data subject, and I give my explicit consent with my free will.
